n. a fancy and redundant way of saying "will." Lawyers and clients like the formal resonance of the language. Will and testament mean the same thing. A document will be the "last" will if the maker of it dies before writing another one. (See: will)

Once the document is created, amend the affidavit as follows: Following the language in the first paragraph of the affidavit, "signed and executed said instrument as his [or her] last will and testament in the presence and hearing of the witnesses," insert "and that he [or she] stated that said instrument is his [or her] last will and testament" and a comma.
