An aortic crossclamp is applied just below the innominate artery, and cardioplegic solution is administered. Instead of cardioplegic solution, which sustains the heart tissue by temporary paralysis, an untrained clerk in the hospital pharmacy--a 19-year-old previously employed as a dry cleaner and an ice-cream store clerk--sent the surgeon plastic bags filled with glucose and water. Based upon what is known about the numerous cardioprotective effects of adenosine, we undertook this program to explore the possibility of modifying existing cardioplegic solutions with the addition of adenosine in an attempt to potentially reduce myocardial stunning and improve patient outcomes. |
