The result is usually a complex combination of manual and automated redundant records including patient profiles, medications Kardex in nursing, medications administration records (MARs), and billing records that are seldom in exact agreement.
Though some of the numbers are not in exact agreement, there is no other self-consistent explanation that anyone's been able to provide.
