(5.) Axelrod TS, McMurtry RY: Open reduction and internal fixation of comminuted, intraarticular fractures of the distal radius.
Anatomical end result was excellent or good in 90% cases, fair or poor in 10% cases and these were intraarticular fractures. Loss of radial length and intraarticular fracture were associated with fair or poor result.
Range of fracture extends from simple extraarticular to highly communited intraarticular fractures. Nearly 16% of all fractures that are treated in emergency departments involve the distal end of the radius.

As a routine for all intraarticular fractures, CT scan is done with 3D reconstruction.

Open reduction and plating is a very good option to treat comminuted intraarticular fractures of the distal radius.
More recent case series have included floating elbow variants including ipsilateral diaphyseal humeral fracture with proximal ulna fracture, and proximal radioulnar joint disruption (Monteggia fracture); ipsilateral diaphyseal humeral fracture, elbow dislocation, and diaphyseal ulna and radius fracture; and distal humerus fractures with intraarticular fractures of the olecranon or radial head [5,7].
The distraction plate technique also uses the concept of ligamentotaxis and, like external fixation, is especially indicated for intraarticular fractures with small, comminuted fragments which may be difficult to manage with plates or other nonbridging methods; when distraction fails to obtain adequate reduction, the use of bone grafts, K-wires and supplementary screws are included in the procedure.
